 📌 Documentation & Letters We Provide

Our licensed therapists are experienced in preparing a wide range of mental health documentation, including:

• WPATH Letters – for gender-affirming medical care and procedures

• FMLA (Family & Medical Leave Act) Documentation – to support workplace medical leave requests

• Short-Term & Long-Term Disability (STD/LTD) Forms – to help you access employment benefits

• ESA (Emotional Support Animal) Letters – for housing and related accommodations

• Psychosexual Assessments – for clinical and legal purposes

• Drug Court & Court-Ordered Probation Assessments – providing professional evaluation and documentation as required

 💵 Fees & Insurance Coverage

• Assessment sessions (the therapeutic appointment required to complete documentation) may be covered by your insurance if we are in-network.

• Preparation of documentation is billed separately and is not covered by insurance.

Documentation fees: 

• With a Licensed Therapist: $125

• With a Lead or NY Therapist: $200

We set these rates to reflect the professional time, expertise, and legal responsibility required to prepare accurate and thorough documentation.
